 What is wrong with my mitt shoulder?

Well i am having problems with my left margin and don't know what to do. I injured it last year in a soccer game when i fell hard on it. I went to the doctors 1 wk later. He said i had a grade 2 margin seperation and put me in a sling. 3 weeks later i went to my orthopedic doctor and he said it was fine. I either well early or i had it for a patch and meet aggravated it. They also said it was because i was skinny (6'0 140lbs). Well it is most a year later now it hurts same a bitch when ever i advise it up and down or lateral to lateral to such and i get this discompose on the top and around the top of my shoulder. Or when i keep it still for to long (ex typing) i will get the same discompose when i advise it up and down or lateral to lateral to much. Also my left margin (injuried shoulder) is most 1/2-1in lower than my right shoulder. I meet looked at my shoulders they are not as baritone as i intellection my left margin the most is 1/2in lower
